All new players must present the documents listed below at registration.

What to bring:

1. Completed Academy Registration Form

2. Completed and Notarized Medical Release form

3. Photo Copy of Birth Certificate

4. Passport Size Photo of Player

Registration will not be considered complete until all items mentioned have been presented to the Academy Registrar.

Academy Fees:

Club Members have three options for fee payment: Pay in full, Pay in installments, or apply for financial asistance. If you would like to pay in the full, then no further documentation is required. If you would like to take advantage of the payment plans, or apply for financial assistance, please complete the Financial Assistance form and contact Kathy Frisbey ay kathyf@ymcaatlanta.org to discuss further.
